At Lions Befrienders, every connection matters. Over the years, we have walked alongside seniors not only through services, but through friendships, shared moments, and everyday support.
As care needs become more complex, no single provider can meet all needs alone. The Integrated Community Care Provider (ICCP) is a sub-region, place-based care model that brings together a network of community care providers to coordinate support more seamlessly for seniors.
Through this network approach, partners work together to ensure seniors are better supported at different stages of ageing—keeping them well, identifying risks earlier, and ensuring timely access to care when needed. The goal is simple: no senior should fall through the gaps or navigate care alone.
What is ICCP?
ICCP is a collaborative network of community care providers within each sub-region, working together to support seniors in a more integrated and coordinated way.
Within this model, partner organisations come together to:
- Keep well seniors socially engaged and supported in the community
- Identify seniors-at-risk earlier and link them to preventive support
- Support seniors with care needs through coordinated assessment and care planning
- Ensure continuity of care across services as needs change
Seniors are supported through a more structured care journey, where providers work in alignment to reduce fragmentation and improve access across services.
A key feature of ICCP is care coordination within each sub-region, where providers collectively ensure seniors receive the right support at the right time, including linking to services both within and beyond the sub-region when needed.
At Lions Befrienders Service Association, we are part of this network and contribute through services such as Ageing Centres (AACs), Senior Care Centres (SCCs), Home Personal Care+, and Home Therapy, delivered in coordination with partner agencies.
With this connected approach, seniors can continue to live well with dignity and confidence in the place they call home.
How Seniors Benefit from ICCP

Through coordinated engagement, seniors with emerging health or social risks can be identified earlier and supported proactively. - More Consistent Care Planning
Seniors receive coordinated assessment and a holistic care plan that evolves with their needs, reducing duplication and service gaps. - Staying Active and Connected in the Community
Through Active Ageing Centres and community partners, seniors remain socially, physically, and mentally engaged within their neighbourhoods. - Neighbourhood-Based, Person-Centred Support
Care is delivered closer to home through sub-regional collaboration, ensuring services are accessible, responsive, and locally coordinated.
Lions Befrienders' role in ICCP
Lions Befrienders is entrusted as the facilitator and collaborator within the ICCP network across four of the seven sub-regions, working closely with community and care partners to support coordination and service integration within each sub-region.
Our sub-regions include:
- Bukit Timah & Queenstown 4
- Clementi 1
- Clementi 2
- Tampines & Paya Lebar 3
Through this role, LB works alongside partner agencies within the sub-regional network to ensure seniors receive more connected, responsive, and person-centred care.
