About HPC+
Our Home Personal Care Plus (HPC+) programme is a step-up of service for vulnerable seniors who require assistance to continue living well at home or need help in supporting caregivers’ duties. Trained care professionals assist the seniors to perform Activities of Daily Living and/or Instrumental Activities of Daily Living, in areas such as maintenance exercises, light housekeeping, personal hygiene, recreational activities, grocery shopping and vital signs check. The programme aims to promote the seniors’ well-being in the comfort of their homes and provide them with access to help whenever needed.
Objectives of hpc+ service
Increase the support to the number of seniors who can age gracefully with dignity at home by offering suitable and timely home-based care options
- Help seniors to age well at home by providing customised care;
- Clients can tap on the modules on needs basis as care needs change/evolve over time for care continuity; and
- Provide higher caretasks and a stronger check-in and monitoring.
Delay/reduce institutional care
- Delay/reduce admissions into acute/long-term institutional care (e.g. nursing homes), by providing more frequent and intensive support at home, reserving nursing home capacity for those with higher care needs and low/no caregiver support.
Refer A Senior
- All referrals must go through Agency for Integrated Care
- All clients must be means-tested
- Fees will be determined after national means testing
Singapore citizens or permanent residents who meet the admission criteria are eligible to apply for government subsidy. The amount of subsidy will be determined through a national means test.
To refer a senior, you may do one of the following:
- Please approach a doctor at any hospital or polyclinic for them to submit an online application on your behalf.
- Contact the Agency for Integrated Care (AIC) at 1800 650 6060, or email enquiries@aic.sg.
- You can also visit any AIC Link branch near you for further support.
